| Approximately 600 youth “age out” of foster care each year and many experience tragic outcomes due to their lack of preparation. To ameliorate these often dire consequences, Minnesota Statutes, section 260C.212, subd. 7 (d) was amended to require the court to review each item of the youth independent living plan and make findings regarding the progress towards, or accomplishment of eleven specific transition goals at the youths annual court review hearing. |
